Обследовано 76 пожилых пациентов с витамин-В12-дефицитной анемией. Путем тетраполярной грудной реографии оценивали состояние центральной гемодинамики, определяли уровни калликреина и прекалликреина в сыворотке крови. Во всех случаях в лечении использовался цианкобаламин в дозе 400-800 мкг/сут. Первые два года заболевания характеризовались усилением сократимости миокарда левого желудочка, его перегрузкой и гипертрофией. Формировался гиперкинетический тип гемодинамики. При длительности заболевания более двух лет развивалась дилатация левого желудочка сердца и формировался гипокинетический тип гемодинамики с нарушением сократимости миокарда. По мнению авторов, наиболее значимой причиной повышения сосудистого тонуса и снижения сократимости сердца является высокий уровень калликреинемии, определявшийся во всех случаях вне зависимости от длительности анамнеза заболевания.
The authors have examined 76 elderly patients suffering from vitamine-В12-deficiency anemia and via tetrapolar thoracic rheography estimated the state of central hemodynamics, defining the levels of kallikrein and prekallikrein in blood serum. In all cases the treatment included Ciankobalamin at the 400-800 mkg/day dose. The first two years of the disease were characterized by en hancing contractility of left ventricular myocardium, its overload and hypertrophy. There was hyperkinetic type of hemodynamics. Given the long duration of the disease lasting over two years, the course of the disease was being characterized by dilation of left ventricle of heart and hypokinetic type of hemodynamics associated with myocardial contractility disorders. The authors consider the high level of kallikreinemia detected in all cases irrespective of the duration of the disease to be the most important cause of an increase in vascular tone and decrease of heart contractility.
