Материал и методы. Обследованы 63 пациента со II-IV стадией злокачественного новообразования. У 41 человека диагностирована анемия (1-я группа), еще в 22 случаях признаков малокровия найдено не было (2-я группа). Выполнен сравнительный анализ показателей обмена железа, С-реактивного белка (СРБ), гепцидина, sTfR, а также интерлейкинов (ИЛ) 6 и 10 и фактора некроза опухоли-а (ФНО-а). Результаты исследования. У пациентов с анемией в сравнении со 2-й группой в сыворотке периферической крови определены более низкие уровни железа, общей железосвязывающей способности, коэффициента насыщения трансферрина железом и более высокие концентрации СРБ, гепцидина, sTfR, ИЛ-6, ИЛ-10 и ФНО-а. Обсуждение полученных данных. На основе корреляционного анализа продемонстрирован многокомпонентный генез анемии при злокачественных новообразованиях. Предлагается рабочий вариант классификации анемии хронических заболеваний на основании ведущего патогенетического фактора: 1) преимущественного дефицита железа, 2) нарушения регуляторных механизмов эритропоэза, 3) недостаточной продукции эритропоэтина.</p></abstract><trans-abstract xml:lang="en"><p>Objective: The study objective is to determine the role of cytokines, hepcidin and soluble transferrin receptor (sTfR) in anemia pathogenesis in patients with malignant neoplasms and to suggest a working version of the pathogenetic classification of anemia of chronic diseases. Methods: 63 patients with stage II-IV of a malignant neoplasm were examined. Anemia (1st group) was diagnosed in 41 people, in 22 cases no signs of anemia were found (2nd group). A comparative analysis of the metabolism of iron, C-reactive protein (CRP), hepcidin, sTfR, as well as interleukins (IL) 6 and 10 and tumor necrosis factor a (TNFa) was performed. Results: Patients with anemia compared with group 2 in the serum of peripheral blood showed lower levels of iron, total iron binding capacity, transferrin saturation coefficient with iron, and higher concentrations of CRP, hepcidin, sTfR, IL6, IL10 and TNFa. Conclusions: Based on the correlation analysis, a multicomponent genesis of anemia in malignant tumors was demonstrated. A working version of the classification of anemia of chronic diseases based on the leading pathogenetic factor is proposed: 1) primary iron deficiency, 2) disruption of the regulatory mechanisms of erythropoiesis, 3) insufficient erythropoietin production.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>злокачественные опухоли</kwd><kwd>анемия</kwd><kwd>обмен железа</kwd><kwd>цитокины</kwd></kwd-group><kwd-group xml:lang="en"><kwd>cancer</kwd><kwd>anemia</kwd><kwd>iron metabolism</kwd><kwd>cytokines</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Анемии / под. ред.
