Анализ литературы указывает на отсутствие единых воспроизводимых критериев для определения показаний к хирургическому вмешательству, что затрудняет прогнозирование исходов при сопоставимой катамнестической эффективности различных методов оперативного воздействия. Современные подходы комбинируют субъективные оценки симптомов, морфологические и функциональные показатели, однако ни один из них по отдельности не обладает достаточной дискриминационной способностью. Перспективным направлением является интеграция аналитической уродинамики и нейрофизиологических представлений о координации детрузора и сфинктерного аппарата для более точной стратификации риска и персонифицированного выбора тактики лечения.</p></abstract><trans-abstract xml:lang="en"><p>Selection of candidates with benign prostatic hyperplasia (BPH) for surgery remains an unsolved clinical task due to the lack of uniform, reproducible criteria. This impedes prediction of outcomes with comparable follow-up effectiveness of different surgical methods. Evidence synthesis indicates that subjective symptom scales, morphological descriptors, and functional parameters each have limited discriminative value when used in isolation.
