Оценить влияние степени тяжести внебольничной пневмонии (ВП) на клинико-функциональные особенности течения и исходы развившегося на фоне этого заболевания острого коронарного синдрома. Материалы и методы. В ретроспективное наблюдательное исследование с элементами дизайна «случай – контроль» было включено 90 пациентов с острым коронарным синдромом (ОКС) в сочетании с предшествовавшей его развитию ВП. Результаты. На моделях регрессионного анализа показано, что гипоксемия (SpO2 &lt; 90%), показатели фибриногена и тропонина T, гемоглобина и тромбоцитов крови в комплексе со степенью тяжести пневмонии и сывороточного С-реактивного белка (СРБ) оказывают основное влияние на скорость струи трикуспидальной регургитации (ССТК), определяя 83,5% значений ее вариабельности. С увеличением ССТК, степенью тяжести пневмонии, приемом диуретиков был связан риск пароксизма фибрилляции предсердий. Заключение. Установлена роль гипоксемии и биологических маркеров воспаления в развитии правожелудочковой сердечной недостаточности, пароксизма фибрилляции предсердий у пациентов с ОКС в сочетании с ВП.</p></abstract><trans-abstract xml:lang="en"><p>Objective: To evaluate the impact of the severity of community-acquired pneumonia (CAP) on the clinical and functional characteristics, course, and outcomes of acute coronary syndrome that developed in the context of CAP. Materials and methods. A retrospective observational study with a case-control design element was conducted on 90 patients with acute coronary syndrome (ACS) and a history of CAP. Results. Regression analysis models revealed that hypoxemia (SpO2 &lt;90%), fibrinogen, troponin T, hemoglobin, and blood platelet levels, in combination with pneumonia severity and serum C-reactive protein (CRP), significantly impact tricuspid regurgitant jet velocity (TRJV), explaining 83.5% of its variability. The risk of paroxysmal atrial fibrillation increased with higher TRJV, greater pneumonia severity, and diuretic use. Conclusion.
