The history of discovery and application of the drugs to treat chronic cardiac insufficiency has been presented. Evolution of the views on the classification and the treatment of chronic cardiac insufficiency has been described. New classification of American Cardiologist College (ACC) and American Heart Association (AHA), 2001 based on the appearance of structural heart changes and prophylactic approach of treatment has been given.

The review of the literature devoted to mechanisms of action of methotrexate and prospects of chemotherapy of tumors. Prospects of development of chemotherapy and biotherapy seem to be optimistic in the situations when the potential of the surgical method of treatment of tumors seems practically exhausted. Constant intensive research in clinical and experimental oncology raises hopes for the further progress in this area and increase in an arsenal of struggle with malignant tumors.

Despite of successes of a farmaceutical industry in development of drugs for treatment of socially-significant not epidemic diseases, death rate from these nozology continues to remain high. It occurs because the treatment of this nozology as process of purpose of medicinal therapy now remains subjective which depend from the age and experience of the doctor influence, and what pharmaceutical representatives visit is more often concrete hospital.

The article shows the problems of introduction of pharmacogenetic researches of systems of biotransformation and carriers of medications in clinical practice for an individualization of pharmacotherapy. On the basis of results of own researches the methodology of studying of clinical value of pharmacogenetic researches of system of biotransformation and carriers of medications is suggested, the basic requirements to pharmacogenetic researches for introduction in clinical practice are formulated.

The results of the calcium alginate effect at the experimental toxic injury caused by four-chloride carbon, at rats are shown. It is shown, that polysaccharide promotes reduction of the aminotranspherase activity, decrease in bilirubin concentration and intensity of processes of the peroxide lipids oxidation that presented by reduction of level of products of the peroxide lipids oxidation in blood and liver of laboratory rats.

The group of patients in amount 45 person received the standard combined therapy by preparations of recombinant IFN-α in combination with ribavirin. The positive effect of standard antivirus treatment at patients with chronic HCV-infection associated with increase of level of IL-2, decrease of levels of TNF-α, IL-1, IL-12р40, IL-12р70 and IL-10 in peripheral serum. It has allowed to use them as noninvasive markers of the steady virologic answer during treatment of a chronic HCV-infection.

The results of studying of dynamics of circulating immune complexes at patients with hemorrhagic fever with renal syndrome are shown. The activity of process of formation of complexes depends on a clinical variant of illness only in early terms and reaches the maximal values by the 3rd week of disease. It is found, that in the feverish period of illness the circulating immune complexes of big size prevail. The dynamics of circulation of an antigen of a virus in blood is investigated as well as in structure of immune complexes. It is shown, that along with decrease in the maintenance of hantavirus antigen in blood by the period of early recovery there is an essential increase of frequency of detection of specific antigen in structure of circulating immune complexes.

The key principles are shown; allowing developing and introducing in clinical practice the standard nutritive therapy in critical condition. In the article the estimation of various approaches to calculation of the requirements of the patient is suggested, efficiency of various methods of parenteral and enteral nutrition by positions of evidence-based medicine is analyzed, the algorithm of realization of a method of nutritive support is discussed at critical conditions.

In the work the data about pathomorphosis of the experimental pseudo-tuberculosis infection under the effect of tinrostim (immune active peptide, activating the system of mononuclear phagocytes) are shown. On the guinea pigs model it is shown, that tinrostim as monotherapy caused reduction of bacterial count, decrease in intensity of circulation changes in inner organs, stimulated macrophages and reparative reaction with cellular detrit resorbtion in the centers of an inflammation. Nonpureblood mice were introduced tinrostim in a combination to gentamycin and it considerably reduced expressiveness of the necrotic component of pathological process and septicopyemia, and also the phenomena of secondary immunodeficiency, promoted the development of immune morphologic reactions in inner organs.

IL-1β, IL-4, IL-6, IL-8, IL-10, TNF-α were measured in 130 pregnant women with different severity of gestosis. It was established, that the production of proinflammatory cytokines is increased and the rate of antiinflammatory cytokines is decreased in the patients with gestosis in comparison with women with physiological pregnancy. The severity of disorders of the cytokine status are in proportion with the severity of gestosis.

Parameters of a mineral metabolism in 130 children, living in Primorsky Region are investigated. In 20 cases osteopenia syndrome was diagnosed. Risk factors of a bone osteoporosis in children are found: age of 12–14 years, family anamnesis, the low level of calcium in the food, insufficient physical activity, and smoking.

Results of local immunity researching in female patients with salpingitis and oophoritis are presented after antibiotic therapy. Clinical and immunological substantiation of Viferon II is created for administration of pharmacologic protocols. Examination capacity and treatment effectiveness control is proposed to increase the quality of pharmacotherapy at the out patient stage.

The analysis of 129 cases of undescended testis is done. The greatest number of references (45%) was in age till 2 years old. Active treatment, conservative therapy and surgical intervention allow to restore anatomy and to normalize testis function. By present time we managed to solve many problems which in other Russian regions have not yet received due development. As a result of preventive actions decrease in amount of cases of infertility at men is achieved.

With a view to establish the natural laws in changes of parameters of some cytokines (IL-2, IL-4, IL-8, IFN-γ) in children with Yersinia pseudotuberculosis infection in dependence of current of the disease 54 patients were examined. The obtained results revealed the hypercytokinemia with disorders of pro- and antiinflammatory pools of cytokines ratio, that proved the presence of cell-mediated immunodeficiency. The results of treatment of patients with recurrent infection by recombinant interleukin-2 (Roncoleukin) were discussed.

Experimental work on 36 white nonlinear rats at whom defect in a belly wall closed with ECBIC film (connecting element, bio-compatible, impregnated by chinoxidine) is done. The analysis of the received results allows to conclude that on initial terms of implantat healing there is aseptic inflammatory reaction of tissue to its introduction with the subsequent replacement by a dense connecting tissue on structure similar to aponeurosis. Surgery was done to 6 patients with hernias. The long-term follow-up results are investigated in 5 years, relapse of hernias is not revealed.

Authors analyzed 273 cases in the age of 9 months – 17 years with an orthopedic and maxillofacial pathology for the risk factors of development of postoperative infectious complications. The found risk factors were used for improvement of approaches to perioperative antibiotic prophylaxis. For an additional estimation of quality of antibiotic prophylaxis the pharmacokinetic researches of used antibiotics (cephasolin, cefuroxim, ceftriaxone) are done. On the basis of the received data the algorithms of the differentiated approach to antibiotic prophylaxis at children with orthopedic and a maxillofacial pathology are developed.

The materials proving expediency of use of biologically active additive «Tinrostim» at a stage of preoperative care and in case of development of postoperative complications at onco-gynecologic patients for prophylaxis and correction of immune system changes are shown. The most informative are determined immune markers of postoperative complications allowing monitoring the severity of complications (level of CD4 +-lymphocytes, phagocytes parameter and phagocytes number of neutrophils) and to estimate the efficiency tinrostim in treatment and prophylaxis.

Influence of biologically active additive «Kalifen» on parameters of lipid spectrum of blood and antioxidative activity at patients with psoriasis of middle severity is investigated. Under the action of medication the level of free cholesterol has decreased at the expressed tendency to stabilization of the etherificated cholesterol that is of great importance for blocking the processes of epidermis’ proliferation in the centers of injury. The natural complex from a guilder-rose positively influenced the changes in phospholipid structure of blood, restored a level of malone dialdegide and the general antioxidative activity of blood. The expediency of inclusion of «Kalifen» in complex therapy of patients with psoriasis is shown.

The frequency of complications (abortions) is analyzed on the basis of the Regional Center of Mother and Child. 78 women who were provided the invasive methods of prenatal diagnostics are surveyed. According to the authors, the frequency of complications of these diagnostic methods did not exceed general parameters.

The action of low-molecular DNA from salmon sperm and the drinks enriched with this substance is investigated, at experimental pseudo-tuberculosis. At prophylactic feeding of the animals by lDNA and drinks, containing it, the survival rate of mice at infection of them with 100% lethal doze of bacteria raised. DNA stimulated the functional activity of peritoneal neutrophil cells and prevented the lethal action of the thermos resistant toxin of Y. рseudo-tuberculоsis. The use of DNA resulted in correction of pathogenic influence of the thermo resistant lethal toxin on oxidizing and antioxidizing enzyme systems of mice’s neutrophil cells.

The nowadays situation of market relations development in Russia’s economy, bring forward a number of problems in pharmaceutical enterprises’ performance, connected with the importance of providing their competitiveness. Given that the choice of strategic development with account of internal and external environment, within which the enterprise operates, gains the most important significance. In Primorsky Region the complex multilevel system of diagnostics and prophylaxis of congenital anomalies and hereditary diseases is introduced$ and it is carried out at three stages: before pregnancy, prenatal and postnatal. All stages are realized by three levels: the first – treatment-and-prophylactic facilities of the region, the second – regional medico-genetic consultation and the centers of planning of family and a reproduction, the third – federal centers.

Main objective of the operative controlling is creation of such control system, which helps effectively to reach the current purposes of the organization and optimizes the cost-benefit ratio also. One of the basic methods of operative controlling is АВС analysis – a way of estimation and accounting of expenses, a way of definition and the account of expenses by kinds of activity of the organization. Modern management of drugs purchases in the health care system based on formular system. АВС analysis allows receiving a complete representation how the formular system works at all of its levels, starting from structural division and finishing by health care as a whole.

The first experience of surgical correction of congenital heart diseases at children of Primorsky Region in the age of 2 days – 17 years old is submitted. 71 surgeries were provided to 69 patients. Postoperative complications are registered in 7 cases, postoperative mortality was 7.2%.

The article is devoted to research of efficiency of the synthetic immunomodulating medication «Polyoxidonium» at various acute allergic diseases. The medication was used together with the basic therapy in age dosages. The estimation of results was carried out in view of clinical and laboratory parameters. Research has proved high efficiency of Polyoxidonium use at immune compromised patients with allergic diseases.

Netromycin has been used for treatment of inflammatory diseases of urinogenital sphere at 36 patients. The technique of the indirect endolymphatic introduction is the next: the medication was introduced into projection of the superficial (external) inguinal ring, in a doze of 200 mg, serially in both sides, once a day, after day, at acute conditions combining it with intramuscular introduction after day; and as a result the patient received Netromycin daily 1 time a day. The clinical effect of the indirect endolymphatic therapy by Netromycin was found already during the treatment course. Control urine was sterile or contained low number of bacteria. Thus, this clinical research have shown, that application of this medication is possible and easily tolerably by indirect lymphotrope introduction.
