EDITORIALS
This article is dedicated to the history of formation and development of one of the largest multifield hospitals of Primorsky region – Municipal Clinical Hospital №2 (Vladivostok). The authors place a great emphasis on the questions of financial provision of medical service under the conditions of formation of market economy model. The authors show a progress in the activity of several services and departments of the hospital, as well as in making of specialized treatment-and-diagnostic centres, in international cooperation. Furthermore, they set priorities of further development of the hospital underlining the upto-date aspects of diagnostic, medical, financial and juridical work to satisfy the needs of the population.
LECTURES
This study represents a review of the literature dedicated to the Still-s disease of adults. On the basis of the analysis of literary sources and findings of his own investigations, the author examines the features of the clinic and diagnostics of this infrequent disease, analyses its epidemiology and current differentially diagnostic criteria, depending on the affection of organs and systems. The fundamental clinical and laboratory symptoms of the disease are adduced, as well as the diagnostic schemes of different authors.
The survey represents a lecture dedicated to up-dated firearm trauma and damaging characteristics of advanced weapons systems. The author examines thoroughly the mechanisms of injuries of different tissues and organs, as well as some adaptation and regeneration processes developing in these structures. Concurrently, a great attention is focused on the analysis of experience of armed conflicts of the centuries XIX-XX. The author draws an inference that the distinctive features of presentday firearm injuries are their multiplicity, associated character and simultaneous lesion of various parts of organism.
REVIEWS
In this article the author brings up for discussion the findings of original experimental and clinical investigations that witness to nitric oxide participation in course of realization of effects of pharmacological preparations. By using of the model of multilevel regulation of respiratory function, the physician demonstrates the participation of central and peripheral nitroxydergical mechanisms of action of adrenoreactive and cholinoreactive medications by means of different isoforms of nitroxyde synthase.
ORIGINAL RESEARCHES
In these researches there was given an analysis of treatment outcomes of 572 patients with liver injury. The authors examined thoroughly the clinic and diagnostic features of 316 closed organ injuries and 256 cases of open ones. The traumas entailed a high lethality (16,1%), and frequently (28,3%) the patients had severe complications thereafter. A special emphasis the authors placed on indications and technique used to perform various operations. An adequate tactical task solution and choice of intervention subject to the characteristics of liver injuries and its intraorganic structures favour, in their opinion, the improvement of the treatment results.
For the first time on the territory of Primorsky region the authors carried out the analysis of sickness rate and clinical course of tick-borne encephalitis of children. Most of the diseased were children from 4 to 12 years. The infection had occurred not only because of tick sting but also after using goat or cow-s milk. At 50.7% of morbid events the disease proceeded as febrilily, at 18% – in the meningeal form and at the remaining ones as focal variants of encephalitis. Under medium-serious forms, in contrast to the serious ones, the disease was characterized by faster normalization of body temperature and reduction of neurologic symptomatology.
These researches show the effect of the twenty most used antibacterial medications on qualitative indices of ejaculate, as well as the content of sex hormones in blood of 243 men of reproductive age. The findings are evidence of the toxic effect of a number of antibiotics on spermatogenesis and hormonal background of the patients participating in the researches.
The author presents the results of surgical treatment of 99 patients with tendon injury of flexor muscle of fingers to whom a tendinous loop suture has been made, and an early medical postoperative rehabilitation has been carried out, according to original protocol during 1992-2002. The programme of functional medical treatment has been realized in compliance with the research results of J.W. Strickland. The outcomes of surgical treatment using loop suture have been traced from 1 till 10 years at 81 patients (146 fingers). The excellent and good results have been obtained in main group at 85,6% of cases, in control group – at 55%.
According to the material of 263 observations, there has been considered an efficacy of various methods of open treatment of urethra strictures. As is shown, the interventions performed with the help of endoscopic equipment (laser urethrotomy, transurethral resection of healing scar tissue) as on a number of indices surpass traditional operations of urethroplasty and bougienage. The authors bring to a focus indications and contraindications to one or another treatment mode of urethral strictures and sclerosis of neck of urinary bladder.
Upon the analysis of late fate of 365 original surgeries and 183 repeated operations, the authors studied the frequency and structure of late complications of aortic femoral reconstructions and detected the dependence of thrombotic complications upon a constellation of indices characterizing peripheral bloodstream capacity, upon the level of total cholesterol, as well as the dependence of emergence of false aneurysm anastomosis upon previous endarterioectomy and arterial hypertension. To improve treatment results and decrease a number of late complications it is of great importance to take into account all these factors under original surgery.
In these researches there was made an analysis of surgical treatment of 174 victims with neck injuries over the last 11 years. The stab and slash wounds had been detected at 93,1% of cases, the gunshot ones – at 6,9%. 101 patients (58%) had vital structures of neck and adjacent anatomical regions damaged. 5 patients died. For the practical purpose it was recommended to divide these patients into two groups: those ones with severe wounds and those with slight and medium-scale wounds. As well, the authors systematized diagnostic and tactical actions under this kind of injuries.
In the article the authors presented an analysis of the clinical signs, first and repeated computed tomographies and outcomes of 7 cases of delayed traumatic intracerebral hematoma. They referred to the delayed ones the hematoma diagnosed with the help of repeated tomographic examination made more than in 24 hours from the moment of the trauma. In 4 cases the delayed hematoma developed after trepanation of the skull. In the researches the authors also discussed the role of microcirculation disturbances and «tamponadeless effect».
The rate of ectopic pregnancy among gynecological patients amounts to 1-6% and tends to become increased. The videolaparoscopic interventions were performed for 65 women aged 17-42, and in 63 cases there were carried out the organ preserving operations. The late fates of the patients were traced at 20 persons and in 4 cases a normal pregnancy came and was carried.
The manifestations of vertebrogenic diseases over the last years became diagnosed and treated with the use of universal method – manual therapy. A simplified notion of back pain pathogenesis is the reason of diagnostic pitfalls, complications and unjustified medical treatment. The objective diagnostic techniques, in author-s opinion, are present-day X-ray examination technologies. According to their results, it can be chosen a rational treatment mode.
In this article the authors offer a policy of examination and treatment of patients with complicated spinal column trauma in the circumstances of time-urgent neurosurgical department. The practical use of the suggested approach in Municipal clinical hospital No. 2 of Vladivostok in 187 cases allowed improving considerably the treatment results of this serious category of patients. A wide range of spinal column operative interventions had been used, and by the treatment outcomes, satisfactory and good results were attained as to 155 persons.
The authors studied 273 cases of chronic tonsillitis and ascertained the distinctions between general and local immune disturbances under various forms of the disease and presence of metatonsillar pathology. The use of immunocorrecting therapy in the complex treatment of the patients allowed eliminating immune system defects and improving clinical course of metatonsillar pathology, as well as achieving during a period of 2-3 years absolute sanation of palatine tonsils and normalization of immunological indices.
In this article the authors examine the method of effectiveness increase under the treatment of crus bone fractures by means of postoperative rehabilitation along with supervised early tensometric weight bearing. They have shown that this type of weight bearing allows controlling and correcting the crus fracture consolidation, as well as normalizes the blood supply, prevents the development of osteoporosis and contractures, shortens rehabilitation dates.
In this article there was carried out the analysis of levels of electromagnetic radiations emitted by transmit-receive and navigation radio equipment on the sea crafts of different models. Inside the enclosed space of the crafts an electromagnetic environment can be considered as safe enough. Outside, on weather deck, electromagnetic radiations can exceed the maximum permissible levels in 1.5-3 times and sometimes in 12-15 times that demands of working out of special measures to insure electromagnetic safety.
In the article the author analyses the experience of treatment of 1056 patients with open hand and finger traumas for whom there have been performed 1533 restorative surgeries. With regard to 761 cases (72.1%) the interventions have been made in process of primary surgical treatment of wound, 416 operations have been carried out using microsurgical equipment. The retrospective analysis of consequences of hand injuries of 126 patients allowed detecting and classifying diagnostic, tactical and operative-and-technical errors influencing upon treatment outcomes. The author has made an in-depth analysis of the stages of operative intervention, has pointed to the tasks of each of them, in detail and sequentially has set forth the techniques used to realize the possibility for recovery of damaged structures.
During the experiment performed on 51 human bony shin segments and in the clinic at the time of treatment of 252 patients the authors examined the fibular bone stabilizing value under single-level shin fractures. As a result, it turned out, that the osteosynthesis of both bones is characterized by strength characteristics that are higher by 31,3-35,3% under axial load and by 19,1-21% under rotary load than osteosynthesis of only shinbone.
On the basis of clinical analysis of 78 surgical observations, the authors prove the necessity of multimodal approach to postoperative analgesia provided for the blockade of painful conduction at all levels – from peripheral to central ones. They evaluated the efficacy of analgesia by ketonal combined with epidural introduction of morphine and showed a high effectiveness of the combined use of the medications under a minimum of complications and side effects.

METHODS
On the basis of 47 observations of their own and analysis of literature, the authors examine advantages of diagnostic and treatment arthroscopy under injury of knee-joint cartilage and note that this method produces better results than radiography and nuclear magnetic resonance. They point out the optimal terms for arthroscopic interventions under knee injury in up-to-date clinic.
This study describes principal phases and diagnostic techniques of detection of the disease stages as well as shows up criteria of objective and subjective effect of the tumor treatment, indications for monotherapy, highest possible androgenic blockade, their advantages and the most frequent complications.
In these researches there was given a description of a technique of preventive temporary clipping of carrier vessel applied for three patients with aneurysms of anterior cerebral – anterior communicating arteries. The duration of clipping varied from 3 to 11 minutes. It was used at the time of dissection and/or clipping of aneurysm. After operation in all cases there were no ischemic complications.
Having made the gastrofibroscopic, morphologic, microbiological examinations of the patients undergone a course of treatment including preparation «Liquid concentrated product of Bifidobacterium», the authors detected little positive dynamics of morphology of mucous coat of stomach and essential distinctions of microbiocenosis and, correspondingly, the indices of children-s health whose treatment had been carried out with use of indicated preparation.
To make ready 22 patients with tumorous affection for the large intestine operations they were applied macrogol 4000 (fortrans) and pinaverium bromide (dicetel). The presence of circular obstructing tumor accompanied by acute intestinal obstruction was considered as a contraindication to use of fortrans. 17 patients had the intake of fortrans combined with the dose of dicetel. The complex use of the medications facilitated some improvement of the quality of preoperative preparation.
MEDICAL HISTORY
The author sums up the forty-five year scientific work of the microbiology, virology and immunology department of Vladivostok State Medical University. A special attention is focused on the showing of 7 stage blocks of the research studies, their tasks and achievements not only in the domain of contribution to science but also from the standpoint of manpower training of higher qualification (12 Doctors of Science and 89 Candidates of Science). The consecution of the stages is genetics and variability of microbes, immunogenetics of diseases and nonspecific resistance of organism, immunogenetics and immunology of feral nidal infections, sea immunology and biotechnology, ecological microbiology and immunology, microbiology and immunology of drug dependence and drug-associated infections.
